qmp: Introduce blockdev-change-medium

Introduce a new QMP command 'blockdev-change-medium' which is intended
to replace the 'change' command for block devices. The existing function
qmp_change_blockdev() is accordingly renamed to
qmp_blockdev_change_medium().
